TRS ActiveCare Application and Change Form 2009/2010 - http://www2.sintonisd.net/forms/trs_application2009-10.pdf Section 6: All Sinton ISD employees must complete the Enrollment Application. For Employees with No changes: Complete only Sec 2 and Sec 10 Indicate “No Changes” at the very top of the Form. For Employees Declining Coverage: Complete Sec 2 and Sec 9. On Sec 9, list employee name and all eligible dependents that are declining coverage. For Employees with Changes: Complete Sec 2, Sec 4, Sec 5 ( if applicable) and Sec 10 Please return completed applications to Anita Martin, via intercampus mail.
